My recommendation is to focus on getting settled and doing well in your classes for the first 3-4 months of first year up to the first year in general. To expand upon that, if your #1 derm program is categorical, would you still send a LOI to your number #1 prelim/TY program that is ranked #1 for your subsequent derm program rankings that are advanced programs?Every program requires minimum of 3 letters, as in the fourth is optional but you must upload at least 3. Some programs will explicitly state that they only accept 3 or that they will not accept a fourth letter.
